Let's get to know each other...

The secure online medical and dental questionnaire will help us to do this. We ask you to complete it to the best of your knowledge. Once completed, click on the Submit button to send it to us. If you prefer, you can complete this questionnaire by hand at the clinic the day of your appointment, in that case, please plan on arriving 20 minutes before the time of your appointment.

IMPORTANT: Once you have completed the form, don't forget to click the Submit button below the last question to send us your answers.

Let's talk dental insurance...

If you have dental insurance, please read the information below and remember to bring proof of insurance to your first appointment so that we can include it in your file.

If you have private dental insurance and your insurer allows it, we can offer you the option to submit your claims electronically for your dental treatments in order to receive your reimbursement as soon as possible.

You are responsible at all times for tpying your tretments in full at the end of each appointment. We accept payments by debit card, credit card (Visa and Mastercard) and cash.

The relationship between you and your insurance is confidential, we recommend that you check the following points with your insurer, as we cannot do it for you:

  • what is the maximum annual allowance for your dental treatment?
  • how often are dental examinations (code 01200) and prophylaxis (code 11300) performed?
  • what is the number of annual dental scaling units available?
  • what is the percentage of coverage for minor dental treatments?
  • what is the percentage of coverage for major dental treatments?

PLEASE NOTE that treatments of crowns, bridges and occlusal plates (night guards) that involve laboratory fees will be sent to your insurer by mail and not electronically. In this case, our secretary will be happy to provide you with the claim document as well as a copy of the laboratory invoices and a post-operative X-ray (which your insurer needs to be able to proceed with the reimbursement). Reimbursement will again be made directly to you according to the terms of your insurance contract.

On the day of the appointment...

IMPORTANT: If your doctor has prescribed a medication (antibiotic or other) for your dental appointments, make sure you take it before you come.

  • Please send us or bring us all your recent dental x-rays (less than 5 years old) in order to limit the number of times you need to take them during this appointment.
  • Carry with you the list of medications you are currently taking, you can get it from your regular pharmacist. Also carry with you any occlusal plates you use (for sports or against bruxism).
  • We ask you to arrive 20 minutes before your appointment time in order to have enough time to complete the opening of your file with your treatment coordinator. She will take this opportunity to show you around the clinic and introduce you to the team members.

On behalf of the entire team, thank you for your trust and welcome to your dentist in St-Bruno!
